Nothing is known about Betsy's life before or during the outbreak. It is likely that she lived near Alexandria before the apocalypse.
After Aaron had found David while on a run, the two encountered Betsy on their trek back to Alexandria. Betsy quickly befriended David and later became his wife approximately 3 months before the Wolves attacked Alexandria.
Betsy is shown to have survived the assault made on the community by The Wolves and is among the townspeople gathered at the front gate, listening to the growing horde of zombies outside the walls. She is in shock and stares at the wall as she listens to Rick and Aaron addressing the crowd. After listening to the speech, she and two other women paint the names of those lost during the assault and the compromised redirection of the walkers from the rock quarry to the wall panel serving as a memorial. Later, as Jessie Anderson walks by her house, she notices something and approaches the door. Suddenly, a zombified Betsy appears at the door and Jessie realizes that Betsy slashed her wrists, allowing herself to die. She is then put down by Jessie as Olivia, Denise, Bruce, Tobin, Barbara, Anna, and other residents watch on with horror.

Depressed over the death of her husband, or because she thinks it's the end over the hordes at the gates, Betsy kills herself by slashing her wrists.

Jessie finds a reanimated Betsy in her home, and puts her down by stabbing her through her eye.

- Betsy is incorrectly credited as Betsey while Talking Dead and David's note in "Thank You" spells her name as Betsy. The name of her counterpart in the Comic Series is also spelled as Betsy.
- Betsy, along with Tommy, are absent from the memorial wall, despite being Alexandrians who recently died.
